By scheduling or receiving services from Midwest DNA, LLC, you agree to the following terms. Clients must provide accurate information and valid government-issued identification at the time of service. Payment is due at the time of service unless prior arrangements have been approved in writing. All sales are final once services are rendered.

Turnaround times are estimates and may vary depending on laboratory processing and regulatory review. Midwest DNA is not responsible for delays caused by third-party laboratories, shipping carriers, or incomplete documentation.

Legal DNA testing follows strict chain of custody requirements and results are released only in accordance with applicable laws and authorization.

Midwest DNA does not provide medical or legal advice, and clients are responsible for consulting qualified professionals regarding interpretation of results. Midwest DNA reserves the right to update these terms at any time.
